The United States Citizenship and Immigration Services (USCIS) requires an i-693 medical exam for anyone looking to get a green card. What Is a USCIS Medical Exam For?
The USCIS medical exam ensures that all immigrants are healthy and current with all mandated vaccines. A person cannot get a valid green card without passing through an adjustment of status. This requires completing a USCIS medical exam on the 2019 version of Form I-693. This I-693 medical examination must be administered by a USCIS certified doctor. These approved medical doctors are called Panel Physicians when they are outside of the US and are called Civil Surgeons if they reside within the US.
Make sure if you’re looking to get a USCIS medical exam you research first whether or not the medical doctor on staff is a USCIS doctor or not. You can search for Civil Surgeon Locator on the USCIS website. Immigration officers will check the credentials of the immigration doctor who signed your I-693 Immigration form. If the medical doctor was not a designated Civil Surgeon, they will send your I-693 form back so that you can re-do it and submit form I-693 again. Please don’t waste your time (or money) on an exam that isn’t going to be valid to obtain a green card.

What to Avoid When Taking a USCIS Medical Exam?
We know that you may research a few options when looking for the place to take your USCIS medical exam, but not every facility has your best interest in mind. Here are a few red flags to look out for when you think you found a deal that might be too good to be true.
1. Any office that promotes an all-inclusive flat fee including all tests (RPR, gonorrhea test for immigration, USCIS TB test), and all adjustment of status vaccinations. Vaccines are not one-size-fits-all, you need to get a specific dose administered by a professional.
2. If they require anything but original, verifiable documents only. Avoid any possible hold-ups by only using supporting documents that are true and are verifiable (such as government records of vaccinations).
3. Immigration doctor offices that don’t do several USCIS medical exams per week. The I-693 immigration form requirement is constantly changing and you need to work with a facility with a trained staff that can get your form filled accurately and completely. The USCIS Tb Test requirements have changed in the past two years and they now require a specific immigration gonorrhea test.
4. Places that mention insurance covering the cost of USCIS medical exams. Immigration-related medical expenses aren’t normally covered and you can call your health plan about this. Any small mix up can result in a denial of your green card or your citizenship can be revoked in the future for healthcare fraud. That’s a big mistake you don’t want to make.
5. Mentioning that the USCIS medical exam can be done in one or two days. It takes at least two visits as there are blood tests and urine tests that take up to 3-4 days when performed by a national lab such as Sunrise or Quest Lab. Any office that does it quicker may not be using a proper lab company or doing a bedside test to fill the form. This is a mistake. 6. Any attorney office that mandates you only use the medical doctors they are sending you to. An immigration medical exam involves a few steps. The Green Card medical exam to any USCIS certified civil surgeon involves two visits as the lab takes a few days to report the results.

The HIV test is not required anymore. The USCIS released a new 9-page I-693 form in 2016. As of August 01, 2016, the USCIS requires a special Gonorrhea Test on all adults above age 15. As of Oct. 1, 2018, the USCIS is mandating the IGRA for tuberculosis testing instead of the PPD test. The IGRA blood test for TB (Quantiferon) is expensive and we can get it done at the most reasonable cost compared to anywhere else in NYC, NY, NJ, or CT. Also, the USCIS is updating its forms once again as of October 1, 2018. So, just in the last several months, there has been a slew of changes in this process.
Yes. You need to let us know that you are pregnant. This is important as certain vaccines and procedures cannot be done during pregnancy. There are antibody levels that we can do in lieu of the vaccines.
If you don’t want vaccinations, you are required to file separate waiver paperwork with USCIS. Once they allow you the waiver, bring that document with you. The Civil Surgeon will review it, fill the individual waiver request, and attach that waiver evidence with your I-693 Form. In almost 30 years of combined experience doing the I-693 forms, only one successful immigrant brought that waiver to us (in our Brooklyn Office). It is that rare of an occurrence, in our experience. In every other situation, your refusal has to be documented on the form, and a notation made that the applicant does not meet the immunization requirements.
Our Civil Surgeons recommend that you consider getting blood tests done instead. These antibody tests can prove that you have immunity and don’t need vaccinations for that disease.
However, laboratory evidence of immunity is acceptable only for measles, mumps, rubella, hepatitis A, hepatitis B, polio, and varicella as per the latest 2020 Vaccination Technical Instructions for Civil Surgeons published by the CDC which handles all public health-related guidelines in the US.
